  7. A graduate of any high school in the City
  8. Applicant to take IQ exam

FOLLOW THESE STEPS TO AVAIL IT WILL TAKE YOU PLEASE APPROACH
1. Applicant and Parents/Guardian proceed to City Social Welfare and Development Office to apply for scholarship 15 minutes Social Worker assigned in the barangay
2. Interview applicant and parent or guardian 30 minutes Social Worker assigned in the barangay
3. Parent/Guardian fill up the Application Form with corresponding signatures 5 minutes Social Worker assigned in the barangay
4. Social Worker endorsed the filled up A. Form to the BSP Focal Person for scheduling of Final Interview and Orientation 5 minutes BSP Focal Person
5. Orientation 30 minutes to 1 hour Bernardita Bangay
6. Enrollment of the child 30 minutes to 1 hour Bernardita Bangay
7. After enrollment period, child/parent/guardian proceed to City Social Welfare and Development Office for release of school supplies, bag, and uniforms/td> 1 hour BSP Focal Person Bernardita Bangay

2. Assessment of Barangay Social Worker as to client’s eligibility for admission to the Center
  • Barangay Social Worker will conduct intake interview
 30 minutes Barangay Social Worker
3. Referral of Barangay Social Worker for client’s admission
  • Barangay Social Worker will contact the Center Social Worker for pre-admission consultation
 15 minutes Barangay Social Worker
4. Admission of client at the center
  • Barangay Social Worker will accompany client to the center and endorse client to the Center Social Worker

1. Receive and interview walk-in referral of Poor Recovering Drug Dependent discharged from Treatment and Rehabilitation Center for 18 months after care program 30 minutes Encarnita Kadile and Jocelyn Yangco
2. Discuss with the client in preparation of the treatment Plan 1 hour Encarnita Kadile and Jocelyn Yangco
3. Conduct Individual counseling to Recovering Drug Dependent 1 hour Encarnita Kadile and Jocelyn Yangco
4. Conduct Relapse prevention session and fellowship meetings 15 minutes Encarnita Kadile and Jocelyn Yangco
5. Conduct Unannounced Drug Testing Duration of client’s stay at the center Encarnita Kadile and Jocelyn Yangco
